Key Responsibilities: • Case Management: Work closely with the clinical team to assess and manage the healthcare needs of Medicare beneficiaries. Develop and implement care plans that align with Cigna quality standards. • *

Patient Advocacy: Serve as an advocate for our Medicare beneficiaries, ensuring they receive the appropriate care and support throughout their healthcare journey.

Data Analysis: Analyze healthcare data to identify trends, gaps in care, and opportunities for improvement. Utilize data-driven insights to make informed decisions.

• Required Skills:

Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN): You must hold a valid LPN or LVN license in the state of practice.
