?Service Area: Abilene, TX (possible float to Albany and Eastland branch if productivity is low, but unlikely)
?Schedule: Full-time, 28 units per week, 40 hours available
?Shift: Monday to Friday, no on-call; occasional SOC/Eval on weekends for ortho patients
?Compensation: Negotiable based on experience and territory budget
?Benefits: $0.48 per mile reimbursement, medical, dental, vision, and 401k (after 90 days)
?Hours: No guaranteed hours; full-time availability 40 hours per week
Requirements: ?Active Physical Therapist (PT) license in the state of Texas ?Required ?Current CPR certification ?Required ?Valid driver? license, reliable transportation, and auto insurance ?Required ?1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting ?Required ?Previous home health experience ?Preferred ?Proficiency in Homecare Homebase (HCHB) and Oasis ?Preferred
Job Duties:
?Evaluate patients to assess their functional status, mobility, and rehabilitation needs.
?Develop and update individualized treatment plans in collaboration with physicians and the home health team.
?Implement therapeutic interventions tailored to patients with neuromuscular, neurological, and orthopedic conditions.
?Educate patients and caregivers on safe mobility techniques, including transfers, gait training, and the use of adaptive equipment such as walkers, wheelchairs, canes, and braces.
?Monitor patient progress, adjust treatment plans as necessary, and ensure all documentation is accurate and compliant with home health regulations.
?Maintain infection control standards and adhere to universal precautions to ensure patient and staff safety.
?Supervise and coordinate patient care with other healthcare professionals, ensuring all treatment aligns with patient needs and goals.
?Perform occasional SOC/Eval on weekends for ortho patients as needed.
