The Behavioral Health Advocate position requires a unique blend of clinical expertise, communication skills, and empathy. You will work closely with patients, families, and healthcare providers to develop personalized care plans that address the complex needs of individuals living with serious mental illness or substance use disorders.

Main Responsibilities: ? Care Coordination: Provide comprehensive care coordination services to adults and children in the SMI/SUD population, ensuring timely access to medical, behavioral health, and social services. ? Patient Engagement: Foster strong relationships with patients and their families, providing education, support, and advocacy to promote self-management and empowerment. ? Community Outreach: Develop and maintain partnerships with community-based organizations, healthcare providers, and other stakeholders to expand access to care and resources. ? Documentation and Reporting: Maintain accurate and detailed records of patient interactions, care plans, and outcomes, using data to inform quality improvement initiatives.
