Position Summary:

Provides effective, efficient, and appropriate primary care services to EMHD Rural Healthcare Clinics? patients in the clinic setting to serve the community?s healthcare needs.

Requirements

Job Qualifications:

Education: Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners. The employee shall meet the requirements of the Texas Board of Medical Examiners and be licensed to practice medicine without restrictions within the State of Texas.

Personal Job-Related Skills: Strong communicator, articulate, and able to interact with community, providers, payors, patients, families, and employees. Utilizes effective oral and written communication skills. Possesses strong organizational and time management skills, is self-directed, uses critical thinking skills, and provides up to date, evidence-based care.

Licensure, Registry or Certification: Current licensure as a NP to practice in the State of Texas by the State Boards.

? Current DEA certification

? Current Healthcare CPR certification (required)

? Current BLS certification (required)

? Current ACLS certification (preferred)

? Current PALS certification (preferred)

Special Requirements: ? Extensive knowledge of the health and aging process ? Demonstrates practical interpersonal communication skills. ? Brings an innovative approach to work, can ?think outside the box.? ? Takes a team approach in accomplishing treatment goals. ? Possesses strong clinical, interpersonal, and communication skills. ? Demonstrates sound judgment in anticipating problems or adverse outcomes; takes appropriate corrective action. ? Takes initiative to engage patients and other health professionals. Participates in activities that support continued competence as a family nurse practitioner. ? Must be willing to travel between practice locations within the county of Eastland, TX. Must be willing to work extended hour clinics as scheduled in the evenings and/or on weekends.

Experience: 2 years prior work experience preferred.

Physical and Mental Requirements:

While performing the duties of this job, the employee is frequently required to walk, sit, stand, talk, and hear. Bending, stooping, and reaching is required while handling equipment and patients. The lifting, pushing, pulling, and carrying some equipment up to 50 pounds or patients up to 150 pounds may be required. Some circumstances may require assistance in moving items or patients.

Essential Duties and Responsibilities:

Patient Care Services:
? Provide health promotion and disease prevention services.
? Order, perform, and evaluate diagnostic tests.
? Diagnose acute, chronic, and complex health problems.
? Prescribe medications and non-pharmacological treatments.
? Develop and manage treatment plans.
? Follow up and evaluate patient status.
? Maintain patient records.
? Patient, family, and community education and counseling
? Coordinate patient care and make referrals as needed.
? Provides comprehensive health assessment, diagnosis, treatment, or referral services to individuals and families.
? Assesses individual needs; anticipates problems; follows up on concerns.
? Practices evidence-based medicine and adhere to the American Nurses Association Code of Ethics
? Leads and participates in multidisciplinary health care team to develop, implement, and evaluate individualized treatment plans.
? Recommend diagnostic testing and procedures; presents findings and recommendations to referring physicians upon request.
? Consults with physicians or specialists regarding patient treatment plans at a level of complexity that exceeds the Family Nurse Practitioner?s scope of practice.
? Consults with and performs screening of pregnant women.
? Participates in case of management activities when appropriate.
? Completes all required records in accordance with clinic/hospital policies.
? Prepares patients for examination, treatment, and study by physicians or other health care personnel, including assistance with procedures such as injections, venipunctures, etc. in accordance with physician orders.
? Documents all patients records in a timely, accurate and efficient manner that demonstrates knowledge and compliance with regulatory requirements and knowledge of coding and clinical documentation requirements.

Demonstrates behaviors that promote positive customer relations with staff, patients, and visitors. ? Reports to work as scheduled and consistently demonstrate dependability and punctuality. ? Completes work within designated time. ? Always follows Patient?s Rights policies. ? Observes facility policy and procedures for infection control and safety according to regulatory guidelines. ? Meets dress code standards and wears identification when on duty. ? Completes annual education/OSHA requirements. ? Manages and operates equipment safely and correctly. ? Accepts assigned duties in a cooperative manner exhibiting acceptable work behavior and conduct. ? Interacts positively with customers, taking into consideration any special needs related to age, emotional condition, or ability. ? All other duties as assigned by supervisor.
