Responsibilities
? Offer assistance to guests or patients as they go through admissions, transfers, or discharges. Conduct regular evaluations to ensure that their personal care needs are being met based on their wishes and the care strategy in place.
? Monitor the care plans consistently to identify any adjustments in the daily care routine for the resident.
Qualifications
? Prior to recruitment, candidates must have completed the necessary CNA license certification.
? Applicants need to possess a valid CPR certification and fingerprint clearance card.
? Candidates with at least six months of experience as a CNA/LNA are given preference.
? Recent practical exposure to long-term care facilities, skilled nursing centers, or transitional rehabilitation environments
? LTC/SNF/Rehab preferred.
