Duties and responsibilities:
Self-Sufficiency Coordinators engage with individuals seeking to disrupt and permanently break the cycle of poverty by providing long-term case management services focused at moving them off of the public welfare system.
? Engage individuals in a way that reduces barriers to their acceptance of services, including meeting them in the office, in their home, and/or other public locations.
? Partner with other agencies that can provide supportive services which address Social Determinates of Health and other barriers to economic self-sufficiency, using a team approach to help clients reach their goals.
? Provide employment search and long-term case management for enrolled individuals, developing goal plans and staying in regular contact to re-assess what resources are needed to help each client achieve economic self-sufficiency.
? Network, participate, and attend related meetings with other relevant agencies.
? Maintain and update client files, using all assigned databases to track client progress, within three working days from contact.
? Assist with the planning, implementation, facilitation, and/or attendance of special events relating to programming and initiatives in the community.
Requirements: ? Two years of directly related and demonstrated experience, or the equivalent combination of education and experience. ? Ability to successfully pass Act 33/34 and FBI clearances. ? Valid Driver?s License and car insurance with an acceptable motor vehicle report. ? Ability to travel within 3-county service region daily.
Salary:
This is a non-exempt position with a starting hourly rate of $19.04. This position is based on a 35 hour workweek.
