The Nurse Practitioner role involves providing comprehensive patient care in a home setting while acting as the clinical team leader for the home health, palliative care, and hospice services.

Responsibilities
? Conduct thorough patient assessments and work with facility staff to promote preventive care and early detection of health changes.
? Oversee the management of both acute and chronic conditions, including modifying medications as necessary.
? Request and analyze diagnostic tests to guide clinical decisions.
? Create, implement, and adjust treatment plans based on patient needs.
? Engage in quality assurance and continuous improvement initiatives.
? Provide clinical education and training for patients, their families, and facility staff.
? Comply with state-specific nurse practitioner scope of practice regulations.
? Exhibit expertise in the causes, risk factors, underlying mechanisms, and clinical manifestations of acute and chronic illnesses.

Qualifications
? Master?s degree in Nursing or an active RN license.
? Licensure as an Advanced Practice Nurse.
? Nurse Practitioner Board Certification from an accredited certifying organization.
? Preferably credentialed with insurance providers.
? Completion of fingerprint clearance.
? Must have unrestricted prescribing and dispensing privileges with a valid DEA license.
? Driving is an essential requirement; a valid state driver?s license and proof of insurance are necessary.
? A minimum of 1 to 2 years of experience as a Nurse Practitioner is preferred.
? Prior experience with adult and geriatric populations, as well as population health management, is advantageous.
? Excellent communication, organization, and customer service abilities.
? Familiarity with Skilled Nursing and Transition Rehabilitation Centers is preferred.
? Competence in managing a high patient volume with confidence and efficiency.
? Proficiency in using Electronic Health Records.
? Ability to handle sensitive and confidential information, including compliance with HIPAA regulations.
? Self-motivated with the ability to initiate and manage tasks independently with minimal supervision.
